Step 4: Complete Your Consultation Form

Log in to the Lift Nutrition Platform and fill out a thorough intake form to share more about your nutrition, lifestyle, habits, health and objectives.

Step 5: Kickoff Consultation with Your Coach

Your dedicated coach schedules a 30-45 minute virtual meeting (via Zoom) to set your initial goals. We’ll cover:

  • Questionnaires: Cover you questionnaire and touch on main points/obstacles  
  • Custom Plans & Habits: Build your personalized nutrition roadmap with daily actions (e.g., meal ideas, habit trackers).
